About This Position

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program.

You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Qualifications

Basic Requirements United States Citizenship Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited.

A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.

Grade Determinations Supervisory Social Worker, GS-12 Education and Experience One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment.

Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification Individuals assigned as Supervisory Social Workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Ability to independently organize work, set priorities, and meet multiple deadlines. (b) Knowledge of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement. (c) Ability to ensure provision of clinical social work services. (d) Ability to provide staff training and development. (e) Skill in interpersonal relationships in dealing with employees, team leaders, and managers. Preferred Experience: At least one year working with homeless and justice involved populations. Physical Requirements: Must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies. Social Worker must be able to perform primarily light and sedentary duties with occasionally moderate physical demands, exercise patience, and control emotions, with reasonable accommodation, if necessary, without endangering the health and safety of the social worker or others. Incumbent must possess and maintain a current valid state driver's license and local travel to provide community based care to Veterans.

The position requires significant travel in the community, through use of a government vehicle or other individual arrangements, to meet with representatives/stakeholders who provide assistance and support to Veterans in clinics, medical centers, Community Clinics, etc.

This position also requires close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; and repetitive motions of the wrists, hands, and/or fingers.

Major Duties

Total Rewards of a Allied Health Professional Oversees and manages the day-to-day operations of the HPACT team, including data collection, administrative records keeping, and monitoring patient/program indicators. Manages staff workload and productivity, and make adjustments to individual caseloads, as needed. Observes and collects outcome measures and meeting formal and descriptive performance measures.

Prepares reports and statistics for the HJCR Deputy Section Chief, facility, VISN, and national use. Establishes, implements, and maintains referral, screening, and admission procedures for program participants which meet national standards as well as the needs of local medical center services and programs. Conducts and participates in outreach activities including field interviews, assessments and referrals for Veterans experiencing homelessness contacted in the community, shelters, and "on the streets," as well as those who are referred by VA residential programs, medical centers, or outpatient clinics.

Maintains current, and establish new, relationships with community-based partners/service providers, and when approved, represent the HPACT program at community meetings. Assists in planning, establishing, and implementing policies and procedures. Provides direct support to the HJCR Deputy Section Chief and assist with the day-to-day program operations and development.

Serves as the first-line supervisor of HPACT and assist with other areas as assigned. Provides direct supervision to an interdisciplinary team and programmatic support in meeting the needs of Veterans experiencing homelessness on HPACT. Organizes orientation and continuing education opportunities for HPACT team members.

Completes performance appraisals and assign related performance ratings, approve awards, and initiate performance-based corrective action, as needed. Develops and maintains professional standards of practice and educate others about them. Works with other VASLCHCS interdisciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to Veterans experiencing homelessness.

Establishes contacts with and where appropriate, assume membership in local coalitions, housing boards and community programs that provide services to Veterans experiencing homelessness. Facilitates and develops Memorandums of Understanding with community-based and private sector providers emphasizing collaboration in the care of Veterans experiencing homelessness. Monitors the safety of staff working with Veterans experiencing homelessness and implement/update related policies and Standard Operating Procedures.

Assesses Veterans identified as appropriate for HJCR programs, facilitating admission and/or referral to appropriate medical and psychiatric services. Evaluates Veteran situations, abilities, and capabilities, arriving at a reasoned conclusion including level of vulnerability and related prioritization for admission. Assesses risk factors, acuity, and need for services, then develop a preliminary treatment plan with Veteran and significant others.

Interprets data and identify viable treatment options to make referrals to VA and community-based services, including VA benefits and other non-VA entitlement programs.
