VA Careers - Licensed Clinical Social Worker: https://youtube.com/embed/U_xC25QsN0w Total Rewards of a Allied Health Professional The Homeless and Justice Clinical Recovery (HJCR) Program includes the following sections: Community Employment, Health Care Homeless Veterans (HCHV) Outreach, Grant and Per Diem (GPO), Homeless-Patient Aligned Care Team (H-PACT), Housing Urban Development (HUD)-Veterans Affairs Supportive Housing (VASH), and Veterans Justice Outreach (VJO). The program provides services throughout the State of Utah, as well as in Southeastern Idaho and Elko, Nevada. Oversee and manage the day-to-day operations of assigned sections of the program, including data collection, administrative records keeping, monitoring patient/program indicators, managing staff workload and productivity, collecting outcome measures, meeting multiple deadlines, measuring Veteran satisfaction, and meeting formal and descriptive performance measures. Prepare reports and statistics for the HJCR Section Chief, facility, VISN, and national use. Establish, implement, and maintain referral, screening, and admission procedures for program participants meeting national standards as well as the needs of VASLCHCS services and programs. Incorporate screening and admission criteria based on established guidance for program eligibility, prioritization of subsets of the homeless Veteran population, VA/HUD/Public Housing Agency (PHA) requirements, and other policies, regulations, or laws pertaining to program administration. As a leader of the team, provide primary input for participant admission based on structured interviews and clinical assessments. Assists in planning, establishing, and implementing policies and procedures. Serve as the first-line supervisor of specific sections/teams, including Veterans Justice Programs and H­ PACT, and assist with other areas as assigned, providing supervision and programmatic support. Complete performance appraisals and assign related performance ratings, approves awards, and initiates performance-based corrective action. Receive and complete consults requesting services from interdisciplinary team members and other health care professionals on complex, difficult cases, using advanced practice skills and expertise. Provide consultation to interdisciplinary team members and ensure development and maintenance of professional standards of practice and educating others about them. Work with other VASLCHCS interdisciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services. Facilitate and develop Memorandums of Understanding (MOU's) with community-based and private sector providers emphasizing collaboration in the care of homeless Veterans. Evaluate the quality control and evaluation of professional services provided. Implement/update related policies and Standard Operating Procedures. Ensure the staff under their leadership are aware of and adhere to all current directives of VHA. Interpret data and identify viable treatment options to make referrals to VA and community-based services, including VA benefits and other non-VA entitlement programs. Conduct ongoing reviews and appraisals to ensure the accomplishment of objectives and adherence to standards set by governing bodies. Provide orientation and on-going training for team members. Serve as a mentor to other social workers and mental health staff. Provide training to students, residents, interns, and fellows from other disciplines, conducting formal workshops and classes as needed/requested. Participate in and assist Section Chief in strategic planning, set of operational priorities and allocation of resources, engage in quality improvement, monitor and address performance measure outcomes. Work with the Veteran, family members, and/or significant others, to develop psychosocial treatment plans including goals for program participation and clinical treatment.. Communicate confidence in the Veteran's ability and worth, helping them address problems utilizing external support systems as appropriate. Incorporates complex multiple causation in differential diagnoses and treatment within approved clinical privileges. Has knowledge in developing & implementing methods for measuring effectiveness of social work practice and service in the Homeless and Justice Clinical Recovery Program, utilizes outcome evaluations to improve treatment services, and to design system changes.
