About This Position

This Social Work Program Coordinator serves as the Medical Center's Grant and Per Diem Liaison/Case Manager for VA's Health Care for Homeless Veterans Programs, Homeless Providers Grant and Per Diem (GPD) Program and is responsible for monitoring and coordination of services provided by community-based programs funded under VA's Homeless Providers Grant and Per Diem Program. Position is located at the Newington VA Medical Center in Newington CT.

Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.

Master's degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited.

Note: A doctoral degree in Social Work may NOT be substituted for the master's degree in Social Work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a Master of Social Work. Licensed or certified by a state to independently practice social work at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure.

Grade Determinations:

In addition to the basic requirements stated above, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade. Social Worker-Program Coordinator GS-12 Experience: One year of experience equivalent to the GS-11 grade level.

Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.

Demonstrated Knowledge, Skills, and Abilities.

In addition to the experience above, candidates must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program.

Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Physical Requirements: The work is generally sedentary. The work requires intermittent standing and walking, with periods of bending and sitting. The work requires light lifting and carrying of light items (under 15 lbs.) such as charts, notes, reports and binders.

Simple grasping and fine manipulation are required when using a keyboard to access a computer. The work requires the incumbent to drive a government vehicle for conducting visits in the community. Work is typically performed in both a clinical and community-based setting which may expose the incumbent to the communicable diseases common to the Veteran population.

Major Duties

Duties of this position include but are not limited to: Provides case management at GPD funded sites as described in the GPD program rules, regulations, and directives. Monitors the clinical care and assesses the compliance of the programs receiving per diem as described in the grant recipient's original grant application. Coordinates and participates in program site inspections in accordance with the GPD Program rules, regulations, and directives as guidance and forwards completed inspection packages to the appropriate offices.

Actively participates in the GPD conference calls and attends training offered by the GPD National Program Office. Participates in the orientation/teaching of graduate social work students, students of other disciplines, and medical center personnel on the biopsychosocial aspects of homelessness. Works with VA Medical Center multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to Homeless Grant and Per Diem Program Veterans.

Participates in the Social Work 24-hour emergency on call coverage program as needed; providing telephone consultation or onsite social work intervention if the clinician evaluates that to be required.
