About This Position

The Social Work Program Coordinator assigned to the Care Coordination and Integrated Case Management (CC&ICM) model of care is responsible for building networks, case identification through evaluation and data analysis, direct case management services, and direct collaboration with nursing and social work CC&ICM leads. The health care and psychosocial needs of the population served are complex and require a high degree of independence, clinical oversight, and creative problem solving.

Qualifications

Basic Requirements: United States Citizenship: Be a citizen of the United States. (Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.) English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j, this part. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited.

A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the 4 accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.

Grade Determinations: Social Worker (Program Coordinator) GS-12: Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency.

Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.

Applicants must meet the requirements for providing supervision in the state where they are licensed at the advanced practice level and should list these qualifications within the assessment narrative and their resume. Documentation is required if part of your state's requirements for providing supervision for licensure is beyond your licensure only (example: certification of continuing education credits). Demonstrated Knowledge, Skills, and Abilities:

In addition to the experience above, the candidate must demonstrate all of the following KSAs (submission of a document with written demonstration of KSAs is highly recommended with application). Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.

Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice.

Major Duties

Major duties include but are not limited to the following: The incumbent must function with a high degree of independence and knowledge of VHA regulations in managing medically and psycho-socially complex cases. The incumbent is responsible for advance level case management and coordination of care for patients across the continuum of care, including initial assessments, recommendations, VA & community referrals, Hospice/Palliative care, supportive counseling, grief/bereavement for patient's families/friends, care coordination, and discharge planning. Demonstrate a high level knowledge of veterans' benefits and services relating to, but not limited to, special programs, VA funeral & burial services, service-connected compensation and non-service connected pension.

She/he will collaborate and facilitate referrals based upon veterans' needs and eligibility. Independently conduct psychosocial assessments. Develop treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team(s) who are involved.

Possess skill and ability to offer psychosocial interventions and measure outcomes to interventions. Incumbent must possess the knowledge and ability to independently implement treatment modalities, provide counseling or psychotherapy for individuals, families and groups, facilitate discharge planning efforts, mentor and lead social work and case management efforts across the health care system, and model leadership to interdisciplinary teams. Develop and implement methods for measuring effectiveness of nursing and social work practice.

Work as a key team member to promote, create and maintain a safe and welcoming collaborative experience among stakeholders for CC&ICM, including, but not limited to, veterans, VA Caregivers, contractors, VA staff and/or other stakeholders of the program. Demonstrate skillful interpersonal communication practices, to include oral and written communication, with staff, veterans and program stakeholders. This includes managing communication at all levels, effectively managing meetings and group discussions, and effectively working across difference.

Develop and sustain positive and effective interpersonal working relationships with staff, veterans and program stakeholders. This includes intentional development of trust and rapport, as well as extending support and cultivating mutual respect. Serve as a subject matter expert on case management and the CC&ICM model of care.

Engage in collaborative efforts and special initiatives with the program lead, SW Service Chief, and Chief Experience Officer as associated with case management that have a direct impact on patient experience and outcomes. Develop and implement policies and procedures specific to NWIHCS and ensures compliance with local facility, VISN, the Social Work Services/Case Management services, Patient Experience Initiatives, VA Central Office directives and handbooks, as well as Joint Commission, and other accrediting bodies and regulatory standards as needed. Ensure full collaboration with nursing and social work colleagues across the health care system.

Identify Veteran cohorts across NWIHCS with the highest opportunity and highest need, to include development and implementation of a monitoring process for ongoing screening and identified cohort(s). Engage in the development of facility wide CC&ICM education planning for key stakeholders.
