About This Position

This position serves as a provider with prescribing privileges as outlined in the individual's scope of practice for Clinical Pharmacist Practitioner (CPP) in Geriatrics/Community Living Center (CLC). The primary responsibility of the CPP is the provision of evidence-based pharmaceutical care services through both direct and indirect interaction with patients and providers.

Qualifications

Basic Requirements: United States Citizenship: Citizen of the United States. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.

NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy. (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Hold a full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.

May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations:

In addition to meeting the basic requirements, applicants must have one (1) year of experience equivalent to the next lower (GS-12) grade level and meet the following knowledge, skills and abilities (KSAs): Ability to communicate orally and in writing to persuade and influence clinical and management decisions Expert understanding of regulatory and quality standards for their program area Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: Possess specific experience in the Geriatrics and Extended Care (GEC) Current or previous experience (including residency training) that includes direct patient care and comprehensive medication management responsibilities. Have an advanced pharmacy degree (PharmD), PGY1 residency, PGY2 specialty residency, and board certification (BPS) or equivalent training and experience providing direct patient care as a pharmacist provider.

Major Duties

Duties include but are not limited to: Customer Service Treats patient, team members, other facility employees and visitors with respect. Responds to the needs of the service in a courteous and timely manner. Demonstrates appropriate and proactive interpersonal skills to achieve the goals of the service.

Acts as a team member and is always supportive of the group's efforts. Demonstrates an ability to work under a variety of circumstances to maximize group productivity and quality. Clinical Provides clinical pharmacotherapeutic/pharmacokinetics services to Geriatrics and Extended Care patients and other medical center patients by consultations and direct patient management.

Provides monthly medication regimen reviews for all Geriatrics and Extended Care inpatients. Establishes and maintains effective interpersonal relations with others which reflect a cooperative attitude. Provides patient care which is appropriate to the cognitive, emotional, and chronological maturation needs of the adult and/or geriatric patient.

Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patient services in his/her assigned service area. Demonstrates knowledge of the changes associated with aging and possesses the ability to provide care based upon age related factors. Obtains medication history from patients and summarizes significant findings in the medical record and/or to the provider as appropriate.

Monitors for and reports drug errors, adverse drug reactions, allergies, and patient compliance issues. Documents findings per facility procedures. Reviews and evaluates requests for non-formulary and restricted drugs for appropriateness and compliance with established criteria where applicable.

Assists with medication use evaluations and other Pharmacy and Therapeutics activities. Documents clinical interventions in CPRS in a timely and professional manner as appropriate. Promotes and monitors compliance for established drug therapy policies.

Reviews and verifies medication orders to be administered to patients in mental health, prior to administration, for appropriateness and to reduce potential risk of adverse drug events. Orders, performs reviews, and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy. Follows-up with patients on lab or test results to discuss the plan of therapy (i.e. changes in medication therapy, monitoring and evaluation, additional testing requirements, referral to primary care or specialty care physician) .

Provides patient care that is appropriate to the cognitive, physical, emotional, and chronological maturation needs of the patients. Administration Participates in quality assurance, performance improvement, drug use evaluation programs and critical pathways as they pertain to pharmacotherapeutics. Serves as a representative of Pharmacy on assigned committees and subcommittees.

Promotes cost effectiveness of formulary. Education and Research Participates in the training and in-servicing of pharmacists, pharmacy technicians, pharmacy students, pharmacy residents, and other medical center personnel as required. Serves as preceptor for pharmacy students and pharmacy residents in accordance with established protocols.

Assists in the development of proposals for improved and/or new clinical pharmacy services. Drug Information Serves as an authority on drug usage, interactions, over-dosages, and compliance to medical staff, nursing staff, technicians, and students. Uses appropriate references to research drug information.
